The following three surveys (series of questions) are those being used by John Jay College, New York, in conducting the research on clerical misconduct with minors for the United States Conference of Catholic Bishops (USCCB).

The Bishop's conference intends to release a report on this information on the first Friday in Lent, February 27, 2004. The information is to be released in the aggregate (the totals across the country) and not by individual diocese, eparchy, or religious order. It will be the option of these entities to release their own specific information if they choose to do so.
